This page contains all documents required for the Senior Project. These documents are to be included in students' Learning Portfolio. Students should always make extra copies of documents submitted for recording and/or grading as back-up, just in case the original is lost.
Students are recommended to store their Senior Project documents in a 3-ring binder to stay organized and to prevent completed documents from getting dirty or damaged. This binder can also be used later as a student's Learning Portfolio.

Petition for Appeal
Use this form to request a due date extension or to appeal an evaluation of a Senior Project component. All appeals are due within three (3) school days of a missed deadline or a failed Senior Project component. Please see Mr. Yoshizawa if you have any questions. |

Senior Project Contract & Release of Liability and Parental Consent
These two forms must be completed and submitted for documentation to Mr. Yoshizawa before a student is considered enrolled in the Senior Project. |

Writing Coach Agreement
All Senior Project students must find a Writing Coach to help them with write their Argumentative Paper. Paper writing help will be provided to students during Student Learning Time. Students should agree on a regular meeting day to ensure they are both available to meet. Once a Writing Coach is found, the Writing Coach Agreement should be completed and turned-in to Mr. Yoshizawa for documentation. |

Mentor Change Form
Fill-out this form and submit to Mr. Yoshizawa to choose a different On-Campus Mentor. Do not wait to change Mentors because you will still need to complete a minimum of 8 meeting logs with your new mentor. |

Senior Project Withdrawal Forms
Can be picked-up from Mr. Yoshizawa. Must be turned-in prior to withdrawal deadline to avoid receiving a "N" grade on their official transcripts.

Students are still responsible for all Senior Project requirements and deadlines until they are officially withdrawn from the program. Students dropped from the Senior Project because of missed assignments will not be allowed to withdraw after having been dropped.
Paper Proposal
Don't forget that your research paper must include an arguable thesis, meaning it should provide you with an opportunity to defend a position. When choosing your topic, also think about how and where you want complete your project hours. |
If you change your topic or your Mentor, you will need to update this document before including it in your Learning Portfolio.

NOTE: It is very difficult to find opportunities in Medical Services and Law Enforcement environments due to privacy and confidentiality concerns. Students are encouraged to talk to their On-Campus Mentor, Counselor, or Senior Project Coordinator about their ideas as soon as possible.
Letter of Intent
This is a letter students write to describe their research topic to the person(s) who will be evaluating their Learning Portfolios and Oral Presentations. Follow the example carefully and be sure to address the letter to "Senior Project Grader." • To be completed by student on own time • Ask your Mentor proofread your letter before turning it in • One copy of your Letter of Intent is to be included in your final Senior Project portfolio. |

Plagiarism Agreement
By signing this document, students and parents acknowledge the responsibilities and consequences associated with creating new knowledge. Completion of the Plagiarism Agreement also requires taking an online quiz and receiving a passing score of 80% or higher. Students may redo the test multiple times to improve their scores. Therefore, students should take the quiz early to ensure having enough time to earn a passing score. |
Be sure to review the tutorial before attempting the quiz. Students must submit a copy of their test certificate which must include their name and score received.

Project Proposal
This document is intended to get students thinking about the four components of the Senior Project and how they relate to the student's chosen topic. If you change your topic or your Mentor, you will need to update this document before including it in your Learning Portfolio. |

Budget Proposal
The Budget Proposal gets students thinking about possible project ideas and the costs associated with their choice. Students should include any costs incurred from their 20+ project hours and implementation of their project. If a student's project hours and project activity does not have any foreseeable costs, simply note "NA" in the 1st row of each column and obtain required signatures. Submit a copy to Senior Project Coordinator BEFORE beginning your project hours. |
• Read the directions and fill-out the proposal form.
• Think about everything you will need to complete your project: transportation, physical items (e.g., folders, printing costs, photocopying, etc.) and any additional costs (even if something you need does not cost anything, list it anyway and state why and how you will be obtaining the item or service).

Advisor Information Sheet
This document explains the criteria for selecting a Project Advisor. It is important for students to get their Advisor's contact information to ensure necessary communication opportunities. This form must be completed BEFORE starting your 20+ project hours. |
If a possible Project Advisor is unwilling to provide any contact information, the student should look for another Advisor since students will need to have documents signed (and maybe resigned), work logs confirmed, student evaluations completed, etc.

Student Evaluation Form
Students should have their Project Advisors complete the student evaluation once their 20+ project hours are finished. This document provides students with valuable feedback on their professionalism and workplace practices. |

Work Logs
Work Logs should be completed regularly (e.g., for each day worked) by the student. The work logs should be very detailed and specific to help the reader understand what the student accomplished during the 20+ project hours. Students' Project Advisor must sign-off on each page of students work logs to verify hours worked |

Mentor Logs
Mentor Logs should be completed each time the student meets with his/her On-Campus Mentor to discuss their Senior Project. Logs should be detailed and describe what was discussed/talked about and what was decided for the next meeting. A minimum of 8 Mentor Logs are required. • Each Mentor Log must be dated and the time recorded. • Logs must be signed by your On-Campus Mentor to verify meetings were conducted. • Signatures (from student and Mentor) are required at the top of EACH new log page. |
